Completing This Form
This form can be completed using an iPhone, Android phone, desktop computer or laptop.
For iPhone users: You can take photos and upload them directly while completing the form.
For Android users: Before starting the form, you must first take all the required photos. Once you have captured the necessary images, you can complete the form and upload them from your phone's gallery. You can upload up to three photos for each item listed below.
Required Photos
Please take clear, landscape-orientated photos of the following:
- Pool – Wide view
- Plant room – Wide view
- Chemical control equipment – Close-up
- Primary sanitation equipment – Close-up
- Secondary purification equipment (if applicable) – Close-up
- Filtration equipment – Close-up
- Water testing equipment – Close-up
If your site has multiple pools or plant rooms, repeat this process for each Pool/Plant Room during the setup.
As part of this setup, you will be required to:
1.Nominate Users
Provide details for up to five (5) authorised users who will have access to the HealthySwim reporting platform for this Site.
2. Provide Equipment Details (Per Pool / Plant Room)
For each body of water, you must provide details of the following systems in operation:
• Dosing / chemical control equipment
• Primary sanitation equipment
• Secondary purification equipment (if applicable)
• Filtration equipment
• Water testing equipment
This information ensures your Site is correctly classified for testing frequency, monitoring, reporting and compliance assessment.
Upload Photos
You will be required to upload clear photos during this setup, including:
• Pool (wide view) (landscape)
• Plant room (wide view) (landscape)
• Chemical control equipment (close-up) (landscape)
• Primary sanitation equipment (close-up) (landscape)
• Secondary purification equipment (if applicable) (close-up) (landscape)
• Filtration equipment (close-up) (landscape)
• Water testing equipment (close-up) (landscape)
Repeat this setup for each Pool / Plant Room at the Site.
